Acute condition in cardiology. Work of a multidisciplinary team

Abstract
According to official data from Rosstat, diseases of the circulatory system occupy the first place in the structure of the population in the Russian Federation. The leading role among acute diseases of the cardiovascular system is occupied by acute coronary syndrome (ACS) and pulmonary embolism (PE). In ACS, the risk of sudden death is high, especially in the acute period, in the first 24 hours of the disease, when mortality can reach 40% of cases. The risk of sudden death from PE is very high, especially with extensive embolism or lack of timely assistance, and can reach 30–50%. However, with adequate and timely treatment, the risk of death is reduced to 8–10%. The principle of a multidisciplinary approach to the treatment of patients with an acute cardiac condition is rapid diagnosis and treatment of the patient, using an integrated approach with the participation of various specialists. The Main Clinical Hospital of the Ministry of Internal Affairs of Russia is the leading healthcare institution of the Ministry of Internal Affairs of the Russian Federation for the provision of specialized medical care in urgent conditions by a multidisciplinary team of specialists of various profiles. This paper presents a clinical case of massive subtotal pulmonary embolism in an employee of the Ministry of Internal Affairs in the intensive care unit and specialized cardiology department using modern laboratory and instrumental techniques, an integrated approach of a multidisciplinary team consisting of cardiologists, surgeons, X-ray surgeons and resuscitators. Thanks to the well-coordinated algorithm of actions of specialists in the employee of the Ministry of Internal Affairs, it was possible to exclude possible complications, prevent a possible fatal outcome and optimally use the treatment and diagnostic capabilities.
