Microbiota in “dry mouth” syndrome among elderly individuals

Abstract
Introduction. Hyposalivation, recently referred to as “dry mouth” syndrome, is common among elderly individuals. Therapeutic and preventive measures for dental rehabilitation in patients with hyposalivation may be ineffective, requiring appropriate patient preparation. The aim of the study is to investigate the microbiota among elderly patients suffering from “dry mouth” syndrome. Materials and methods. A microbiological study using PCR diagnostics for periodontopathogens (Prevotella intermedia, Bacteroides forsythus, Treponema denticola, Actinobacillus actinomycetem comitans, Porhyromonas gingivalis) was conducted on 33 elderly patients with «dry mouth» syndrome, divided into 2 groups based on the degree of salivary dysfunction, in comparison with a control group (15 individuals). Results. It was found that in the control group, the studied periodontopathogens were detected in 20.0–26.67% of patients. With a decrease in unstimulated saliva output from 0.5 ml to 1.25 ml in 5 minutes, an increase in the occurrence of the studied periodontopathogens by 40.0–53.33% was noted. When the unstimulated saliva production was less than 0.5 ml in 5 minutes, the studied periodontopathogens were found to be 64.62–80.0% more frequent than in the control group of patients, despite the absence of inflammatory processes in the periodontal tissues and oral mucosa. Conclusion. In elderly individuals with varying degrees of salivary dysfunction, periodontopathogens such as Prevotella intermedia, Bacteroides forsythus, Treponema denticola, Actinobacillus actinomycetem comitans, and Porhyromonas gingivalis are detected in 60–100% of cases, which must be considered when prescribing rational therapy and performing dental rehabilitation, taking into account the existing changes in oral microbiota.
