Registry of patients with short bowel syndrome and some remote results of their dynamic observation

Abstract

Introduction. Short bowel syndrome (SBS) is a clinical symptom complex of digestive disorders caused by a decrease in the absorption surface of the small intestine and manifested by intestinal insufficiency of varying severity (maldigestion and malabsorption). Many issues of routing and rehabilitation of this category of patients remain unresolved in most regions of the Russian Federation. Objective — to create a registry of adult patients to assess the main clinical and anatomical manifestations of SBS and their need for infusion support and parenteral nutrition. Materials and methods. The data from the created database of the registry, which included 156 adult patients with (SBS living in various regions of the Russian Federation, were analyzed. Results. A survey was conducted in 51 patients with (SBS). Of these, the majority of patients (20 people — 39%) had a residual segment of the small intestine from 51 to 100 cm, less than 50 cm — 15 people (29%). Among the surveyed patients, 39 people (76.5%) received parenteral nutrition (PN), and 36 people (70.6%) of them also received infusion therapy (IT). The majority of patients who need PP receive it on their own at home (25 people — 49.0% of the respondents), only 3 people (5%) had to be hospitalized periodically to carry out PN and IT. 11 people (21.6%) have been in hospital treatment since the surgery, which is associated not only with the need for nutritional and metabolic therapy, but also with other types of medical benefits. Conclusion. The data obtained during the survey of patients with (SBS indicate the existing problems of rehabilitation of this category of patients, which is due to the need to prescribe them long-term, and often lifelong, not only PN, but supportive IT in outpatient (home) settings. This requires a systematic organizational approach to routing such patients and the creation of specialized centers that ensure not only the proper implementation of home clinical nutrition, but also monitoring of their condition.
